The patient chart
Updated
The chart gathers everything you know about a patient. Open it from Patients → View, or with quick search (⌘K).
Who can use it: professionals, administrators and owners. Only professionals at the patient's clinic can write in it. Everyone else sees it read-only.
What's on the screen
- Patient details: age, ID number, contact details, sex at birth, insurance and blood type.
- Medical Alerts: allergies and chronic conditions, if recorded.
- Latest Vitals.
- Tabs:
- Clinical History: the notes on the left, and the editor for a new one on the right.
- Appointments: every appointment the patient has had.
- Vitals: the history of measurements.
- TimeLens: clinical images, if the clinic has the module on.
- Odontogram: if the clinic has the module on.
- Team Notes: internal notes that aren't part of the clinical record.
- Shortcuts: File, Vitals and Team Note.

Good to know
- If the patient doesn't use the patient portal yet, Invite to portal appears at the top: it emails them a link to create their account. Once invited, the button reads Resend invite. It disappears when the patient creates their account.
- What you write from the chart is saved without an appointment. To link it to a visit, write it from the consultation (see The consultation).
- TimeLens and the Odontogram are turned on per clinic in Settings → Booking Settings → Consultation modules.
Troubleshooting
| Problem | What to do |
|---|---|
| "Read only" | Only professionals at the patient's clinic can write. Ask your administrator to add you to that clinic. |
| "Patient not found" | They may belong to another clinic, or have been deleted. Check the clinic selected in the top bar. |
| TimeLens or Odontogram is missing | The module isn't on for this clinic. |
